This excel workbook was developed to assist in capturing Ebola-related preparedness and response actions over time. The workbook was initially developed for use only at the local health department level to identify which of the CDC 15 Public Health Preparedness Capabilities were addressed in response to actions taken regarding Ebola. The workbook has since been expanded to include the ASPR 8 Healthcare Preparedness Capabilities to allow for complete documentation across all levels of the healthcare coalitions' preparedness and response activities. The workbook is divided into sections. The first section relates to the CDC 15 Public Health Preparedness Capabilities. A separate worksheet is devoted to each capability, and allows for the documentation of any activities taken under a particular function, and whether associated performance measures have been met or not. Further, the document allows the ability to capture any strengths, areas for improvement and resultant action items. The second section relates to the ASPR 8 Healthcare Preparedness Capabilities. Similar to the Public Health Capability worksheets, the Healthcare Capabilities also have worksheets devoted to each capability. The intent behind the creation of this tool is to be able to show documentation of all activities taken, and to serve as a resource for completion of multiple local, state and federal deliverables across the public health spectrum.
