Collaborative Practice Agreements (CPAs) are legal documents that define collaborative practice relationships between pharmacists and physicians. CPAs are used in areas where pharmacies are required to have physician oversight when providing certain clinical services. These services can include immunizations, point-of-care testing (POC), collaborative drug therapy management, etc. This document assists clinicians in creating CPAs, but can be adapted by local health departments. For instance, CPAs can be developed for public health emergencies. In some jurisdictions, CPAs can be made between the state health officer or medical director and qualifying pharmacies/pharmacists; such CPAs typically go into effect only during public health emergencies. This document includes a standardized, easily customizable format for a CPA. The nine-part CPA can be brief or extensive depending upon the clinical service to be provided and the needs and desires of the collaborating parties.
