Limited health literacy is associated with poor health outcomes, specifically related to poor management of chronic disease. Local Health Departments can use this toolkit to develop clear communication practices and improve public health services for all women, children, and families in their jurisdictions. This toolkit grew out of the the Agency for Healthcare Research and Quality, which commissioned the University of North Carolina at Chapel Hill to develop and test the toolkit.
