Franklin County Health Department (FCHD), located in Frankfort, KY, serves a population of 49,000 and has 100 FT, PT and contract employees. FCHD served as a PHAB beta test site and received technical assistance from NACCHO around quality improvement as an add-on to the beta test. They developed their QI plan by using the QI Plan of the Tacoma-Pierce County Health Department as a template.
