This toolkit provides an improved framework for engaging community leaders, clinicians, families, and Title V directors to assist children and youth with special health care needs (CSHCN). The materials included in this toolkit were collected by learning collaboratives that were funded by the Health Resources and Services Administration's Maternal and Child Health Bureau. Each collaborative identified and tested a number of tools that families and providers could use to support quality improvement within systems of care for CSHCNs. Examples of communication tools include the Family Newborn Hearing Screening Roadmap, care notebooks, and a pictorial description of seizures. Accessibility: This website is generally screen reader accessible, but forms may be difficult to use, links may not be labeled, and images may not have alternate text. Diverse tools and resources are provided on this website. PDFs may not be fully accessible using a screen reader or Read Out Loud.
