Project Overview: The Mercer-Putnam-Sullivan Public Health Regional Collaborative consists of the Mercer, Putnam, and Sullivan County Health Departments, which serve a combined population of 15,552 in North-Central Missouri. The collaborative identified gaps in strategic planning and developed a community health improvement plan to address these gaps. The plan will also improve the capacity to communicate consistent messages about public health, health promotion, and disease prevention to the public.
