CSTE recently published a National Strategic Plan for Chronic Disease Surveillance Modernization, which details six core strategies that public health partners can implement to leverage a modernized information ecosystem. The strategic plan lays the foundation for an integrated and enhanced chronic disease surveillance infrastructure and is designed to align with broader federal efforts around public health data modernization, including the CDC’s Public Health Data Strategy (PHDS) and the priorities described by the National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP) 

Modernizing chronic disease surveillance is crucial to effectively use data to identify and reduce the burden of chronic diseases in all communities. The complete plan can be accessed here. Its structure, objectives, and goals can be tailored to your Local Health Department’s needs.
