A cover sheet/transfer form and associated discharge packet for patients with an MDRO leaving a nursing facility. It includes contact precaution signage, a letter from the Epidemiology Program manager explaining the outbreak, enhanced standard precautions guidance, detail about where a specimen was collected and why, and health department staff contact information. Used in conjunction with other materials developed as part of a containment toolkit.
