This tool contains 2 templates. The first template, developed by the New York Office of Mental Health, guides public health agencies in the development of a quality improvement (QI) plan taking into consideration the many components of a QI plan including: mission and vision; leadership; goals and objectives; performance measurement; and performance indicators. The second template, designed by MarMason Consulting to include the essential components of an agency QI Plan. These are excellent resources for those LHDs with more advanced-level experience with QI that are ready to infuse a culture of QI within the agency.
