NACCHO, in partnership with LTG Associates, Michelle Reiss-Top of Mission Economic Development Agency (MEDA), and Ariana Anjaz and Reza Pedram of Afghan Health Initiative (AHI), presented the Closing the Referral Loop Webinar. A challenge for many non-profits, CBOs, and health agencies is being able to track what happens once a client/patient is referred to another agency for services. This information is important to individual service provision, to service system integration, and to being able to gauge and demonstrate services process and outcomes. In this webinar, our panelists addressed the methods and systems that they use for tracking service referrals.
