One of the drivers of suboptimal infant feeding and early breastfeeding discontinuation is gaps in the continuity of care (CoC) in lactation resources and support within communities. To achieve CoC, families should have access to effective delivery of lactation resources, support, and healthcare services. To address these gaps, NACCHO and the U.S. Breastfeeding Committee (USBC)’s Continuity of Care Constellation launched the Continuity of Care in Breastfeeding Support: A Blueprint for Communities on August 24, 2021.
With support from the CDC Division of Nutrition, Physical Activity, and Obesity (DNPAO), NACCHO is pleased to announce a funding opportunity to support the implementation of the CoC Blueprint to strengthen community lactation support. Applicants are required to apply with at least two key partners to implement strategies from at least three recommendations from the CoC Blueprint. Applicants can also select additional activities to improve the community’s child nutrition security infrastructure.
NACCHO will select up to five local organizations that will each receive an award of up to $50,000.

Application Instructions

NACCHO will also host an informational RFA webinar on September 21, 2023, at 1:00 PM ET. Please register for the webinar here.
