These facility notification templates can be used to notify healthcare facilities when there is a suspect case of carbapenemase-producing (CP) carbapenem-resistant Enterobacteriaceae (CRE). There is a template for a positive (confirmed) case, as well as for a case that is negative for (CP-CRE) but positive for CRE.
