COVID-19 has had a profound impact on our society beyond the physical toll on those infected with the virus. It has also caused restriction of resources for non-COVID conditions, economic upheaval, and interrupted care on chronic conditions.
NACCHO is pleased to announced the release of three Request for Proposals to support partners’ program evaluation, continuous quality improvement (CQI), data application, communications, partnership development, racial and health equity, and sustainability efforts related to employing SDoH approaches to improving chronic disease.
