In partnership with the Arizona Department of Health Services, the Coconino County Health Department administers nutrition and physical activity programs throughout the state. One particular program targets Native American communities on the Hopi and Navajo reservations. Working closely with tribal governance, the program aims to build community health capacity while reducing obesity-related morbidity, including type II diabetes and cardiovascular disease. The program employs a community health worker model and recruits and trains health representatives from the community that act as liaisons between the community and the health department to develop activities and craft messages that are relevant to the community.
