The Northwest Colorado Visiting Nurse Association is building an integrated community health system that includes prevention and wellness services, primary care provision, hospitals, assisted living facilities, hospice care, and home health services. The Outreach and Prevention Team's Aging Well Program focuses on wellness and chronic disease prevention among Latino adults through community-based outreach and education, screenings, and medical case management.
