Place Matters addresses the disproportionate burden of environmental hazards and poor health outcomes, including high asthma hospitalization rates and cancer incidence, for residents in the South Valley of Bernalillo County. Bernalillo County Environmental Health partners with the Kellogg Foundation to raise awareness about health disparities and advocate in local government for land-use policies that will protect the community's health. The Health Equity Assessment Tool (HEAT) is an epidemiological surveillance tool based on the CADH Health Equity Index that allows community members and analysts to explore the relationship between social conditions and health outcomes. The community is also engaged through a series of focus groups and town hall meetings that discuss the relationship between land-use and health and develop policy recommendations.
