With funding from the Center for Disease Control and Prevention's (CDC) REACH program, the Genesee County Health Department implemented a community-based participatory research approach to address the disparity in infant mortality experienced by its African American population. With information from community surveys, focus groups, and forums the health department and its community partners developed a community action plan that identified three areas of focus to reduce infant mortality rates and improve birth outcomes. The REACH 2010 Team implemented a Community Action Plan based on a socio-ecological model of health designed to: foster community mobilization, enhance the "babycare" system, and reduce racism. Since the implementation of the REACH project, infant mortality rates have decreased countywide and the gap between rates for African Americans and whites has shrunk.
