Chicago's predominately African American and Latino neighborhoods of North and South Lawndale experience disproportionately high rates of type II diabetes and cardiovascular disease. Targeting at risk residents in these communities, the Chicago Department of Public Health has partnered with residents and more than 63 hospitals, community agencies, schools, and government agencies to offer case management, community capacity-building, outreach, and education to improve the health status of Lawndale residents.
