Accreditation Support Initiative

The Centers for Disease Control and Prevention’s Office for State, Tribal, Local and Territorial Support has provided funding to NACCHO for the Accreditation Support Initiatives (ASIs). The ASIs are intended to stimulate quality improvement and promote the readiness of health departments to seek voluntary national accreditation through the Public Health Accreditation Board (PHAB). Project activities include efforts to meet the identified application requirements, achievement of PHAB standards and measures, and offsetting accreditation fees (up to 50 percent of the total fee). This initiative provided direct, targeted funding to recipients and demonstrated how relatively small amounts of funding can be of great interest and value to health departments in their accreditation readiness efforts. Descriptions of the previously funded sites and their deliverables are linked below.

Accreditation Preparation

The seventh year of the ASI supported 24 local public health departments in undertaking accreditation preparation activities during the project period of December 2017-June 2018. Sites applied for funding of up to $15,000 to complete one or more predetermined deliverables within 1-2 of five categories of work related to organizing for the accreditation process and the PHAB Standards and Measures. The selected health departments represent 13 different states and a wide range of population sizes, jurisdiction types, and accreditation readiness needs.

For the first time, NACCHO, in collaboration with CDC and PHAB, offered a special opportunity for small/rural health departments through which these selected ASI sites received customized technical assistance to support their accreditation readiness efforts.

Category 5 - Building a Culture of Quality Improvement (related PHAB Domain: 9)

Quality improvement (QI) is the result of leadership support and requires staff commitment at all levels to build a culture of quality and ensure it is fully integrated into organizational structures, processes, services, operations, and more. Applicants to this category may use this award to conduct activities that will show progress towards building a culture of QI that will fulfill one or more predetermined deliverables (e.g., assessment of current QI culture and description of desired future state, plan for regularly communicating about QI activities, etc.).

2012-2013 Awardees

In fall 2012, the Centers for Disease Control and Prevention’s Office for State, Tribal, Local and Territorial Support provided funding to NACCHO to renew the Accreditation Support Initiatives (ASI) for a second year. The ASIs are intended to stimulate quality improvement and promote the readiness of health departments to seek voluntary national accreditation through the Public Health Accreditation Board (PHAB). Project activities include efforts to meet the

identified application requirements, achievement of PHAB standards and measures, and offsetting accreditation fees (up to 50 percent of the total fee). These two initiatives, which are providing direct, targeted funding of approximately $1.2 million across 31 total recipients, demonstrate how relatively small amounts of funding can be of great interest and value to health departments in their accreditation readiness efforts. Descriptions of the two initiatives, and the activities of the sites funded through them, are provided below.



This opportunity was available to all public health departments (state, local, territorial and tribal) as well as support organizations that assist local and tribal health departments (e.g., state health departments, public health institutes, state associations of county and city health officials) to undertake accreditation activities. Approximately 130 applications were received, illustrating heavy interest. Twenty awardees were identified to receive funding between $5,000 and $40,000 each:

Through the Affordable Care Act Prevention and Public Health Fund (ACA-PPHF), specific funding was made available to support the accreditation readiness efforts of local health departments (LHDs) serving large metropolitan jurisdictions. Applications to this initiative were solicited by invitation only and the opportunity was limited to health departments not directly receiving funding through the CDC National Public Health Improvement Initiative. Sites in this initiative will benefit from additional networking opportunities throughout the project period. Eleven jurisdictions were selected as awardees, receiving up to $65,000 each:

3. City of El Paso Department of Health (TX)
Funded to update its community health assessment and improvement plan; produce a written roadmap for addressing accreditation requirements; draft a workforce development plan; and complete manuals needed for documentation in Domains 2 and 3. The health department will also provide technical assistance on accreditation to a smaller LHD in their jurisdiction.

4. Columbus Public Health (OH)
Funded to develop processes related to implementation of the community health improvement plan and developing competency-based job descriptions; finalize a workforce development plan and training curriculum plan; and update its quality improvement training plan. The health department will also provide technical assistance on accreditation to a smaller LHD in their jurisdiction, and will develop a start-up kit to help LHDs just beginning the accreditation process. 5. Cuyahoga County Board of Health (OH)
Funded to complete significant portions of the community health improvement process and develop documentation for several PHAB domains. The health department will also provide related technical assistance to a smaller LHD in their jurisdiction.

Funded to develop and implement a functioning performance/quality management system and complete documentation needed for Domain 9.1. The health department will also share these learnings and provide related technical assistance to other LHDs in their jurisdiction.

8. Multnomah County Health Department (OR)
Funded to develop plans and policies for a functioning performance/quality management system; finalize the quality improvement and communications plans; and provide staff and leadership training. The health department will also provide technical assistance on accreditation to smaller LHDs in their jurisdiction.

9. Orange County Health Department (CA)
Funded to complete a community health assessment using the Mobilizing for Action through Planning and Partnerships framework; complete a quality improvement plan; and provide quality improvement training to staff.

10. Public Health-Seattle & King County (WA)
Funded to support development of and implement a workforce training needs assessment instrument; document strategies for engaging staff in workforce surveys; produce draft workforce development and training evaluation plans; and develop a system for tracking staff professional development activities. The health department will also provide related technical assistance to smaller LHDs in their jurisdiction.

2. Iowa Department of Public Health (supporting work of multiple LHDs)
Funded to conduct formative research to identify current practices of Iowa LHDs relating to their performance management systems, and provide technical assistance on how current activities may either be formalized or adjusted to meet PHAB standards. Findings will be used to inform the development of a performance management curriculum for all of Iowa’s LHDs.

3. Missouri Institute for Community Health (MICH)
Funded to provide technical assistance and consulting services, focused on strategic planning and quality improvement, to five Missouri LHDs preparing for PHAB accreditation. The five LHDs include: two rural LHDs that are accredited by MICH, two rural LHDS not accredited by MICH, and one semi-metro LHD not accredited by MICH.

5. North Dakota State Association of City and County Health Officials
In a joint collaboration with the North Dakota department of health, funded to create a public health accreditation infrastructure serving 28 health departments; review PHAB standards and measures and familiarize LHDs with examples of acceptable documentation; support seven LHDs in developing a CHIP; and complete a performance improvement plan.

Through ACA funding, a total of $600,000 was available to support accreditation readiness efforts in large metropolitan jurisdictions. The opportunity was limited to health departments serving populations of more than 1.5 million but not directly receiving funding through the CDC national public health improvement initiative. In addition to the work described below, each LHD must also engage a “connector site,” (i.e., smaller LHD) to assist them in preparing for accreditation. Six jurisdictions were selected as awardees, receiving up to $62,000 each:
